An electric toothbrush produces the strokes so your hand only has to guide them. Two motions dominate the shelf. Oscillating rotating heads are small and round and twist one way then back, usually 7,500 to 8,800 times a minute, cleaning roughly one tooth at a time. Sonic heads keep the familiar brush shape and sweep side to side at 24,000 to 40,000 movements a minute.

Inside the handle sit a motor, a rechargeable lithium cell and a small circuit that counts. Counting is the part that changes behavior: a two minute timer that stutters or stops at the end, and a pacer that pulses every 30 seconds to move you to the next quarter of the mouth. A pressure sensor reads how hard the head is loaded and either lights up or cuts the motor. That is the most useful thing on the handle. Charging runs inductively through a sealed base, so no contacts corrode. A healthy cell covers about two weeks of twice daily use per charge.

Trials comparing powered with manual brushing report modestly less plaque and less gum bleeding after three months, with oscillating rotating heads leading. That gain is real, small, and largest for people whose technique is poor. Arthritis, a shoulder injury, fixed braces and teenagers make the clearest cases. Against a Manual Toothbrush the cost gap is wide. Against Dental Floss or an Interdental Brush nothing changes at all, since no motor reaches between two touching teeth.

Handles are the cheap part of this purchase. Replacement heads run several dollars each and you need about four a year. Five years of heads often costs more than the brush did. Head shapes are proprietary, meaning the handle locks you into one supply for its whole life. Travel adds its own cost. The handle is heavier than a manual brush, and a flat battery in a hotel with the wrong charger leaves you brushing with a freebie.

Choose on soft head availability, a pressure sensor, a two minute timer and the price of a year of heads, then ignore the extra modes, which mostly change rhythm. Guide the head slowly along the gumline and rest it on each tooth for a couple of seconds rather than scrubbing, because scrubbing fights the motor. Rinse the head with the motor running, pull it off weekly and rinse the metal spindle where paste dries, and replace the head every three months.
